Step 1 – Register Account

1. Enter a unique 6 to 15 character. Username for your account.
2. Enter Password. Your Password must be at least 6 characters long. It Must contain at least one lower case letter, one upper case letter, one digit and one special character
3. Re-enter your Password.
4. Enter your primary Email Address.
5. Confirm you have read the Terms and Conditions and Privacy Statement by clicking on the checkbox.
6. Review and confirm that you agree to the Business Associate Agreement by clicking on the checkbox.
7. Press Next to continue.
